Report Discipleship Participation Numbers

 

This form helps the church office report weekly numbers to the Virginia Conference. Report the name of your group, the date you met (each week), and the number of children, youth, young adults, and adults who participated. Choose as many of the "Kind of Event" choices that apply to your group/event. The total number of persons served, refers to those in the community who you reached through your group/event (i.e. Daily Bread, Park View, Easter Egg Hunt, Sing-alongs).
